This form is for the use of professional educators who have successfully completed credits at Philadelphia College of Osteopathic Medicine. Act 48 requires all certified educators, every five years, to earn six college credits, or six continuing professional education credits, or 180 clock hours, or continuing professional education, or any combination of collegiate studies, continuing professional education courses or learning experiences equivalent to 180 hours. Philadelphia College of Osteopathic Medicine has been authorized to submit to the Pennsylvania Department of Education graduate credit courses only.
